Nursing care that comes to you

Nursing services are provided by nursing staff and are shaped by your care plan. Here is what that can include.

  • Help with medication

    Safe administration, prompting and monitoring — so the right dose happens at the right time, every time.

    This can include

    • Administering prescribed medication, including injections and eye drops
    • Preparing and checking dose administration aids (Webster packs)
    • Watching for side effects and liaising with your GP or pharmacist
  • Managing symptoms of medical conditions

    Ongoing clinical support for chronic and complex conditions, delivered in your own living room.

    This can include

    • Diabetes support, including blood glucose monitoring and insulin
    • Continence and catheter care
    • Wound care, dressings and pressure-injury prevention
  • End of life and palliative care

    Gentle, unhurried care that keeps comfort and dignity at the centre — for the person and for the people around them.

    This can include

    • Pain and symptom management in partnership with your palliative team
    • Personal care delivered with patience and respect
    • Practical and emotional support for family and carers
  • Checking overall health

    Regular clinical assessment that catches small changes before they become hospital admissions.

    This can include

    • Observations — blood pressure, pulse, temperature, oxygen saturation
    • Skin integrity, mobility and falls-risk assessment
    • Clear written reporting back to your GP
  • After an operation or hospital stay

    The bridge between the ward and home, so recovery keeps its momentum after discharge.

    This can include

    • Post-operative wound review, dressings and suture or clip removal
    • Drain, stoma and IV therapy management
    • Following the discharge plan and escalating anything that looks wrong
  • Help with day-to-day activities

    Showering, toileting and dressing — the everyday things, handled with privacy and warmth.

    This can include

    • Showering, bathing and personal hygiene
    • Toileting and continence assistance
    • Dressing, grooming and safe transfers
  • Other tasks where there is a clinical need

    If a registered nurse should be doing it and it is clinically indicated, talk to us about it.

    This can include

    • Nursing tasks identified in your care plan
    • Coordination with allied health and community services
    • Care-plan review as needs change

The services listed above reflect the DVA Community Nursing programme. What you actually receive is determined by your referral and approved care plan — if something you need is not on this list, ask us anyway.

What to expect

What a visit actually looks like

No surprises, no rotating cast of strangers. Just a nurse who knows your history turning up when they said they would.

  • We arrive when we said we would

    Appointments are booked around your routine — not squeezed into a run sheet you never agreed to.

  • Clinical care, then the human part

    Observations, dressings, medications — and then the conversation that makes the visit worth having.

  • Everything is documented and reported

    Clear notes go back to your GP and DVA so nothing gets lost between appointments.

Eligibility

Who these services are for

Available to eligible DVA clients holding a Veteran Card — All Conditions (Gold Card) or a Veteran Card — Specific Conditions (White Card).

  • Gold Card

    Veteran Card — All Conditions

    Covers treatment for all clinically necessary health care needs, whether or not they relate to service. Community nursing is available with an approved referral.

  • White Card

    Veteran Card — Specific Conditions

    Covers treatment for accepted service-related conditions. Community nursing is available where the need relates to an accepted condition.
